While injury and even death are common, the greater danger is to one's Path, for defeat can shake one's confidence, ruining the foundations and casting him back into the Elemental Hells. But the opportunity to test one's conviction and power against a peer and reinforce his Path and standing are few and far in between, so few would turn a duel down.
